Qualifications

• LPN/LVN, LSW, LCSW, or LMSW or license other than RN per state law.
• Minimum of 2 years of experience with individuals who have chronic illnesses, co-morbidities, or disabilities in a Service Coordinator or similar role.
• State-specific certifications may be required by state law or contract.

Responsibilities

• Assist the RN in telephonic or face-to-face assessments to identify, evaluate, coordinate, and manage member needs.
• Support care coordination by contributing to care plans and monitoring member needs.
• Coordinate care for members with chronic illnesses or disabilities to ensure cost-effective utilization of health benefits.
