Job summary

Elevance Health is seeking a LTSS Service Coordinator – Clinician (LSW/LCSW/LPN) to support members statewide in Ohio. The role is primarily field‑based with occasional office attendance. Salary ranges from $30.89 to $38.61 per hour, with a Columbus location noted and hybrid/remote considerations per policy.

The incumbent will assist in care coordination across physical, behavioral, and long‑term services, under RN oversight, ensuring person‑centered care and efficient use of health benefits.

Qualifications

• Requires an LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) and minimum of 2 years of experience with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
• Preferred: BA/BS in Health/Nursing and related background.

Responsibilities

• Identify members for high risk complications and obtain clinical data as directed.
• Identify members who may benefit from alternative level of care or waiver programs.
• Provide information to the responsible RN who develops and monitors the member's care plan.
• Coordinate care for members with chronic illnesses in partnership with the RN and health care team to ensure cost effective benefit utilization.
• Make decisions based on current needs and disease knowledge, applying clinical guidelines without nursing judgment.
