Elevance Health is seeking a LTSS Service Coordinator Clinician to operate primarily in the field across Marion and Hendricks counties, IN. This role supports the RN in case management for members with chronic conditions, coordinating care and collecting data to inform care plans.

The position requires licensure as LPN/LVN or social work license per state law, with at least two years in care coordination or similar settings. Hybrid/field work with travel to client sites is expected.

Qualifications

• Licensed practical nurse or social work license per state law.
• Minimum 2 years of experience with chronic illnesses or disabilities in care coordination.
• Current, unrestricted license as applicable.

Responsibilities

• Assist the RN in identifying members with high risk complications and gather clinical data as directed.
• Support care coordination by identifying alternative levels of care or waiver programs as appropriate.
• Provide information to the RN for development, monitoring, and revision of the member care plan.
