Job summary

CVS Health is looking for an Analyst, Case Management Field to support LTSS with field-based duties in the Rock Island County area of Illinois. The role requires 50–75% travel to visit members in person, manage a full caseload, and complete quarterly in-person visits.

The job emphasizes care coordination, education, and use of care management tools to optimize member outcomes, with a strong focus on quality and regulatory compliance. A bachelor’s or master’s in human-services is preferred.

Qualifications

• Must reside in Illinois, Rock Island area and have a valid IL driver’s license.
• Willing to travel up to 75% to meet with members.
• Minimum of two years of case management experience.
• Proficient in Microsoft Office and electronic health record systems.

Responsibilities

• Evaluate referred members’ needs/eligibility and plan appropriate case resolutions.
• Identify high-risk factors and coordinate care management referrals as needed.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers and Medical Directors to overcome barriers and achieve goals.
