Job summary

Molina Healthcare seeks a Community Health Worker to advocate for members and connect them with health services. The role involves in-field visits, home visits, and collaboration with care teams to support vulnerable populations.

The worker helps navigate Medicaid benefits, arranges transportation, and connects clients to social services, housing, and other supports. Travel is extensive and program needs may vary by state.

Qualifications

• At least 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid driver’s license, reliable transportation, and adequate auto insurance for travel.
• Ability to multi-task while speaking with members.
• Excellent customer service and communication skills.

Responsibilities

• Engages with members as an advocate and resource for health care needs.
• Supports health care services team with non-clinical duties in the field (home visits, shelters, provider offices).
• Helps members navigate and maximize Medicaid benefits and other resources.
• Assists members in accessing social services (housing, food, employment, etc.).
• Provides outreach to locate disconnected members with special needs.
• Conducts research to locate hard-to-reach members using data and community resources.
• Participates in outreach activities (telephonic and face-to-face).
• Guides members in maintaining eligibility and accessing financial resources.
