Job summary

Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy. You will act as a local resource to assist vulnerable members in navigating health care needs and benefits, contributing to cost-effective, quality member care.

You will meet with members in homes, shelters, and provider offices, aiding with appointments, transportation, and social services while traveling locally 50-80% of the time.

Qualifications

• At least 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid driver's license and reliable transportation for travel.
• Ability to multi-task while speaking with members.
• Excellent customer service, organizational and time-management skills.
• Proficiency with Microsoft Office and relevant software systems.

Responsibilities

• Engages with members as an advocate and resource to support management of health care needs.
• Collaborates with the health care services team performing non-clinical duties in the field.
• Helps members navigate and maximize health plan benefits and arrange transportation.
• Assists access to social services and community resources.
• Outreach to locate/disconnected members with special needs.
• Performs research to locate hard-to-reach members using available data.
• Participates in outreach activities, telephonic and face-to-face.
• Guides members to maintain Medicaid eligibility and other financial resources.
• 50-80% local travel may be required.
