Molina Healthcare seeks a local Community Health Worker to support member advocacy and connect vulnerable individuals with health and social services. You will travel to members' homes, shelters, and provider offices to assist with navigating benefits and scheduling care.

The role emphasizes non-clinical paraprofessional support, strong customer service, and collaboration with field teams; travel 50-80% may be required depending on state contracts.

Qualifications

• At least 1 year of health care experience with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver's license and reliable transportation for travel requirements.
• Ability to multi-task applications while speaking with members.
• Excellent customer service and communication skills.
• Organizational and time-management skills.
• Willingness to learn other lines of business and software systems.

Responsibilities

• Engages with members as an advocate and resource to support health care needs.
• Collaborates with health care services team on non-clinical paraprofessional duties in the field.
• Helps members navigate and maximize health plan benefits and schedule appointments.
• Assists members in accessing social services and community resources.
• Provides outreach to locate disconnected members with special needs.
• Conducts research to locate hard-to-contact members by leveraging internal databases and community locations.
• Participates in outreach efforts (telephonic or face-to-face).
• Guides members to maintain Medicaid eligibility and other financial resources.
• Local travel 50-80% may be required.
