Qualifications

• Bachelor's degree is required.
• Experience in behavioral health or social work is essential.
• 2+ years of case management experience is preferred.
• Spanish language skills are preferred.

Responsibilities

• Conduct comprehensive assessments of mental health, substance use, and physical health.
• Develop crisis intervention plans prioritizing safety and recovery.
• Coordinate inpatient and outpatient transitions with hospital teams and social work.
• Manage EHR documentation in compliance with HIPAA.
• Facilitate intake for diverse populations with culturally competent care.
