Job summary

Pillars Community Health, LLC is seeking a Case Manager to support the Mobile Crisis Response team. You will assist clients with hospital engagement, discharge planning, crisis stabilization, and linkage to post-hospital and outpatient services.

The role requires coordinating with hospitals and community providers to ensure continuity of care. Qualifications include a Bachelor's in a related field and 1–2 years of case management, with bilingual English/Spanish abilities.

Qualifications

• Bachelor’s degree in counseling, psychology, social work or related field is required.
• 1–2 years of direct client case management experience in social services, behavioral health, or related field is required.
• Customer service experience is required.

Responsibilities

• Meets clients and families in hospitals, in the community, or at their home and coordinates discharge planning and post-hospital referrals.
• Collaborates with hospital social workers and coordinates referrals for nursing services, substance use treatment, behavioral health, and other resources.
• Opens client records, uploads documentation and safety plans, and maintains required chart updates.
