Responsibilities

• Be clinically and culturally competent with training to manage complex cases in the community across child-serving systems.
• Coordinates and implements assigned care plan activities and monitors care plan progress.
• Negotiates options and services to meet member benefits/healthcare needs.
• Guides member through healthcare decision-making and supports empowerment for healthy lifestyle choices.
• Acts as single point of contact for members and helps remediate gaps in care and access.
• Monitors and documents care in compliance with regulations and company policies.
