Job summary

CVS Health in Ohio is seeking a full-time Field-based Case Management Coordinator. The role travels to member homes and other sites up to 50% of the time and requires a Bachelor’s degree or equivalent clinical qualification.

You will coordinate care for children and families within Northeast Ohio, working to improve health outcomes, manage complex needs, and collaborate with care teams across systems.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years in behavioral health, social services, or human services.
• 2+ years experience with Ohio delivery systems and community resources.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate member needs, benefits and available programs.
• Provide coaching and support to empower member health decisions.
• Act as single contact for members and address gaps in care.
