Job summary

CVS Health is seeking a Case Management Coordinator for a full-time, field-based telework role in Ohio. The position requires travel up to 50% within a Northeast Ohio region to member homes and sites, with strong coordination of care plan activities and education for families.

The ideal candidate holds a bachelor’s degree or an equivalent clinical degree and has 2+ years of behavioral health experience, including familiarity with Ohio delivery systems.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years experience with Ohio delivery systems, including local community networks and resources.
• Willing and able to travel within the assigned region up to 50% of the time.

Responsibilities

• Be clinically and culturally competent with training to manage complex cases in the community across child-serving systems.
• Coordinate and implement assigned care plan activities and monitors care plan progress.
• Enhance medical appropriateness and quality of care through collaboration with care teams and providers.
• Monitor, evaluate and document care in compliance with regulatory guidelines and company policies.
• Provide coaching and support to empower members to participate in healthcare decisions.
