Job summary

CVS Health Corporation is hiring for a full-time field-based care coordination role in Ohio. Travel up to 50% within the Northeast Ohio region to member homes and locations is expected. Strong preference for candidates in Marion, Crawford, Sandusky, Summit, and surrounding counties.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related field.
• Willingness to travel up to 50% within the region.

Responsibilities

• Be clinically and culturally competent in managing complex cases across child-serving systems.
• Evaluate members and coordinate care plans and services.
• Enhance medical appropriateness and quality of care through collaboration with health teams.
• Monitor, evaluate, and document care per regulatory guidelines.
