Job summary

CVS Health is seeking a Case Management Coordinator in Ohio for a full-time, field-based telework role. You will travel within Northeast Ohio up to 50% of the time to member homes and facilities, coordinating care and supporting families through the behavioral health services system.

The ideal candidate has a bachelor’s or non-licensed master in behavioral health or related field with 2+ years of experience in children’s mental health and public sector services, plus strong MS Office skills and

Qualifications

Bachelor’s degree or non-licensed master level clinician required.
2+ years of experience in behavioral health, social services, or human services.
2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
2+ years of experience in children's mental health, child welfare, or related public sector field.
2+ years experience in family systems, case management or related areas.
Willing to travel within the region up to 50% of the time; some travel to New Albany may be required.
Reliable transportation; mileage reimbursed.

Responsibilities

Be clinically and culturally competent in managing complex cases across child-serving systems.
Evaluate member needs and coordinate care plans; monitor progress.
Collaborate with health teams to ensure optimal outcomes and motivate engagement.
Provide coaching and support to empower independent health decisions.
Serve as single point of contact to remediate care gaps and access.
