CVS Health Corporation is seeking a Case Manager Analyst - Field to support the LTSS program. This role requires travel to the west side of Chicago and surrounding Will & DuPage Counties, with quarterly in-person member visits and a full caseload management focus.

The ideal candidate will have 2+ years of case management experience, strong Microsoft Office skills, and familiarity with electronic health records, enabling effective care coordination and member education.

Qualifications

• Minimum 2 years of case management experience.
• Must reside in Illinois, west side of Chicago area.
• Valid Illinois driver's license and reliable transportation.

Responsibilities

• Coordinate care and education for LTSS waiver members.
• Travel 50–75% to meet members face-to-face and conduct quarterly visits.
