Moms In Motion / At Home Your Way in Virginia is hiring a Service Facilitator (SF) Case Manager to support families in navigating Medicaid Waiver programs and coordinate care.

The role involves visiting clients at home, coordinating plans of care, and resolving issues like authorizations and timesheets—no degree required, just heart and experience. You can work remotely from your laptop when not visiting clients.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience are valued.
• Tech-savvy for email, docs, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle (no client transport).
• Excellent written and spoken communication; pass a background check with two references.
• Bonus: Person-Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Serve as the go-to guide for families navigating waiver programs.
• Write plans of care and assessments that make a difference.
• Troubleshoot service authorizations, timesheets, and related tasks.
• Build lasting relationships with families based on respect and trust.
• Work remotely from your laptop/tablet when not visiting clients.
