Job summary

Molina Healthcare is seeking a Care Manager to support care management and coordination across the continuum. You will work with a multidisciplinary team to ensure member progress toward outcomes and to deliver quality, cost-effective care.

Responsibilities include assessing members, developing and implementing care plans, and ongoing monitoring. Travel up to 40% may be required based on state/contractual requirements.

Qualifications

• At least 2 years experience in health care, care management preferred.
• Excellent verbal and written communication skills.
• Ability to work independently and manage multiple priorities.

Responsibilities

• Complete member assessments and determine care coordination eligibility.
• Develop and implement care plans with member and care team.
• Conduct telephonic, home visits or in-person meetings as required.
• Monitor care plan effectiveness and document progress.
• Coordinate with ICT meetings and multidisciplinary team.
