Job summary

Molina Healthcare is seeking an RN Care Manager to support care coordination for members across the continuum. You will assess needs, develop care plans, and collaborate with a multidisciplinary team to improve outcomes and reduce costs.

The role requires 2+ years in health care, an active RN license, and strong communication and organizational skills. Travel up to 40% may be required depending on state contracts.

Qualifications

• At least 2 years experience in health care, care management, or related settings.
• RN license active and unrestricted.
• Valid driver's license and reliable transportation for travel.
• Understanding of EMR and HIPAA.
• Strong time-management and communication skills.

Responsibilities

• Complete comprehensive member assessments per timelines and determine eligibility for care management.
• Develop and implement care coordination plans with member, caregiver, physician and care team.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plan effectiveness, document interventions and adjust as needed.
• Maintain ongoing member caseload for regular outreach and management.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team (ICT) meetings and collaboration.
• Use motivational interviewing and Molina guidelines to educate and motivate change.
• Assess barriers to care and provide coordination assistance.
• May provide consultation to peers as needed.
• Care manager RNs may handle complex cases and medication reconciliation.
