Qualifications

• 2+ years in health care, care management or related setting
• Licensure/certification as required by state contract or regulation
• Valid driver's license and reliable transportation for travel

• Strong verbal and written communication skills

Responsabilités

• Complete member assessments within regulated timelines and qualify for care coordination.
• Develop and implement care plans with member and care network to address needs.
• Conduct telephonic, in-person, or home visits.
• Monitor care plan effectiveness and document interventions.
• Maintain ongoing member caseload and outreach.
• Coordinate with ICT and other care teams to integrate services.
• Use motivational interviewing to educate and motivate change.
• Assess barriers to care and provide assistance.
