Qualifikationen

• 2+ years healthcare experience with LTSS/care management.
• Clinical licensure/certification if required by state contract/regulation.
• Bachelor's degree may be required in some states.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and ability to navigate portals/databases.
• Proficiency with Microsoft Office and relevant software.
• Experience with substance use disorders in the past year is a plus.

• Complete comprehensive member assessments within regulated timelines, including home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care network.
• Monitor care plans and adjust interventions to achieve goals.
• Coordinate integrated care and LTSS across settings.
• Assess medical necessity and authorize waiver services.
• Collaborate with ICT for approvals/denials and team collaboration.
• Educate and motivate members using motivational interviewing principles.
• Identify barriers to care and address psycho/social/financial obstacles.
• Engage with licensed care managers as needed.
• Travel 25-40% based on state/contract requirements.
