Qualifikationen

• At least 2 years experience in health care, preferably care management or related settings.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.
• Knowledge of community resources.
• Ability to work independently with minimal supervision.
• Strong communication and time-management skills.
• Proficiency with Microsoft Office.
• In some states, a bachelor's degree may be required depending on state contracts.

Aufgaben

• Completes assessments of members per timelines and determines eligibility for care coordination.
• Develops and implements care plans with member and care network to address needs and goals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors and documents progress toward goals and adjusts care plans accordingly.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guides to educate and motivate change.
• Assesses barriers to care and addresses member concerns.
• Collaborates with licensed care managers/leadership as needed.
• Local travel 25-40% may be required (depending on location/contractual requirements).
