Qualifications

• At least 2 years of healthcare experience, preferably in care management.
• Valid driver's license and reliable transportation.
• Strong communication and problem-solving skills.
• Ability to work independently and manage time effectively.
• Proficiency with Microsoft Office or similar software.

Responsibilities

• Complete member assessments within regulated timelines and determine eligibility for care coordination.
• Develop and implement care plans with member, caregiver, physician and care network.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plans, document interventions and outcomes, and adjust as needed.
• Maintain ongoing member caseload and regular outreach.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assess barriers to care and provide coordination assistance.
• Collaborate with licensed care managers as needed.
• Travel 25-40% locally as required.
