Molina Healthcare is seeking a Care Manager to support coordination across the care continuum. You will work with a multidisciplinary team to assess member needs, develop care plans, and monitor progress toward outcomes. Travel up to 40% may be required based on state contracts.

Required are 2+ years in health care and a valid driver's license; clinical licensure is conditional on state requirements. Compensation includes comprehensive benefits and an EO E.

Qualifications

• 2+ years experience in health care, care management or related setting.
• Knowledge of community resources and ability to navigate referrals.
• Proactive, detail-oriented with strong communication skills.

• Complete member assessments within timelines and identify care coordination eligibility.

• Conduct telephonic or in-person visits and monitor care plan effectiveness.
