Qualifications

• 2+ years in health care with care management or LTSS experience.
• Active license as LCSW/LMSW/APSW/LPCC/LMFT or equivalent.
• Valid driver's license and reliable transportation for travel.

Responsibilities

• Support care management and care coordination for LTSS.
• Collaborate with multidisciplinary team to coordinate member care.
• Complete member assessments within timelines, including home visits as needed.
• Develop and monitor care plans with stakeholders and track progress.
• Facilitate waiver enrollment/disenrollment and ICT meetings.
