Humana Gold Plus Integrated is seeking a Field Care Manager RN to assess and guide members toward resources for optimal wellness. The role involves in-home visits, plans of care, and coordination with healthcare providers to meet patient goals.

Requirements include Illinois RN licensure, 2+ years in-home case management, strong communication, and proficiency with EMR and MS Office. Hybrid home-based with travel across Illinois.

Qualifications

• Must reside in Illinois.
• Registered Nurse in Illinois with no disciplinary action.

Responsibilities

• Identify and resolve barriers to care.
• Monitor patient progress through assessments/evaluations.
• Create plans of care for members.
• Communicate with internal and external stakeholders.
• Understand department and organizational objectives and linkages.
• Make decisions with minimal direction.
• Follow established guidelines/procedures.
• Travel 75–90% to visit Medicaid members in homes or care facilities.
