Requires Illinois RN license, 2+ years in-home case management, and comfort with EMR systems. Travel is substantial, with a typical 40-hour week and possible on-site office in Schaumburg, IL.

Qualifications

• Must reside in Illinois.
• Illinois RN license without disciplinary action.
• 2+ years in-home case/care management.
• Experience with adult populations.
• Knowledge of community health resources.
• Excellent communication and interpersonal skills.
• Ability to work with minimal supervision.
• Experience with EMR and MS Office tools.

Responsibilities

• Identify and resolve barriers hindering care.
• Monitor and evaluate patient progress toward outcomes.
• Create plans of care.
• Communicate with internal and external stakeholders.
• Align work with department and organizational objectives.
• Make independent decisions within scope and guidelines.
• Adhere to guidelines and procedures.
• Travel to visit Medicaid members 75–90% locally.
