Qualifications

• Must reside in Illinois.
• Registered Nurse (RN) licensed in Illinois without disciplinary action.
• 2+ years of in-home case management experience.
• Experience working with adult population.
• Knowledge of community health resources and social services.
• Excellent communication and interpersonal skills; rapport building.
• Ability to work with minimal supervision.
• Experience using electronic information systems, EMR, Word/Outlook/Excel.

Responsibilities

• Identify and resolve barriers to effective care.
• Monitor patient progress through assessments and evaluations.
• Create plans of care.
• Communicate with internal and external stakeholders.
• Understand department strategy and objectives and their linkages.
• Make decisions with minimal direction and follow established guidelines.
• Conduct home visits (Medicaid members) across care settings; 75-90% travel.
