Qualifications

• Must reside in Illinois and be a registered nurse.
• 2+ years in-home case/care management.
• Experience with adult population.
• Knowledge of community health resources.
• Strong communication and interpersonal skills.
• Ability to work with minimal supervision.
• Experience with EMR and Microsoft software.

Responsibilities

• Identify and resolve barriers to care.
• Monitor progress through assessments and evaluations.
• Create plans of care.

• Make decisions with minimal direction.
• Follow established guidelines/procedures.
