Qualifications

• Must reside in Illinois.
• Illinois RN license without disciplinary action.
• 2+ years in-home case management.
• Experience with adult populations.
• Knowledge of community health resources.
• Excellent communication and interpersonal skills.
• Ability to work with minimal supervision.
• Experience with EMR/EHR systems.
• Proficient with MS Word, Outlook and Excel.
• Excellent keyboard and web navigation skills.

Responsibilities

• Identify and resolve barriers hindering effective care.
• Monitor patient progress through assessments and evaluations.
• Create plans of care.
• Communicate with internal and external stakeholders.
• Understand department, segment, and organizational strategy and objectives.
• Make decisions with minimal direction, occasionally in ambiguity.
• Follow established guidelines/procedures.
• Home/field visits for Medicaid members with 75–90% travel.
