Job summary

Humana is seeking a Field Care Manager Registered Nurse (RN) to assess and evaluate members' needs, guiding them toward resources for optimal wellness. The role involves creating care plans, monitoring progress, and coordinating with families and providers in a hybrid home-based setting with substantial travel across Illinois.

The ideal candidate will have IL RN licensure, at least 2 years of in-home care management experience, and strong communication skills.

Qualifications

• Must reside in Illinois
• Registered Nurse in Illinois without disciplinary action
• 2+ years in-home case/care management
• Experience with adult population
• Knowledge of community health and social service agencies
• Excellent communication and interpersonal skills with the ability to build rapport
• Ability to use electronic information applications including EMR
• Proficient with Microsoft Word, Outlook and Excel

Responsibilities

• Identify and resolve barriers hindering effective care
• Monitor patient progress through assessments and evaluations
• Create plans of care
• Communicate with internal and external stakeholders
• Understand department, segment, and organizational strategy and objectives
• Make decisions with minimal direction when needed
• Follow established guidelines and procedures
• Visit Medicaid members in homes, Supportive Living Facilities and Long-Term Care Facilities; 75–90% local travel
