Job summary

Humana Gold Plus Integrated is seeking a Field Care Manager Registered Nurse (RN) to assess and evaluate member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members and families toward resources appropriate for the care and wellbeing of members. The role includes planning and coordinating care with stakeholders and traveling within Illinois.

The position requires IL RN licensure, 2+ years of in-home case management, and strong communication.

Qualifications

• Illinois residency and Illinois RN license without disciplinary action.
• 2+ years in-home case/care management; experience with adults.
• Knowledge of community health resources; strong communication.
• Ability to work with minimal supervision and use EMR software.
• Excellent keyboard and web navigation skills.

Responsibilities

• Assess and evaluate member needs to guide resources for care.
• Create and manage plans of care; monitor progress.
• Communicate with internal and external stakeholders.
• Visit Medicaid members in homes and care facilities; 75–90% travel.
• Work within TB screening and driver safety programs.
