CTC seeks a Field Care Manager Nurse 2 based in Michigan, traveling 75% to Wayne or Macomb counties to conduct in-home visits for Medicare/Medicaid members. The role focuses on managing physical, environmental, and psycho-social health issues with an interdisciplinary team.

You will develop care plans, coordinate with ICT, and ensure access to community resources, aiming for optimal member outcomes. This is a full-time position with local travel.

Qualifications

• Active Michigan RN license with no disciplinary action.
• Must reside in Michigan.
• Ability to travel to homes and community settings for face-to-face assessments.
• Experience with adult population and disease management.
• Knowledge of community health and social service resources.
• Excellent communication and interpersonal skills.
• Ability to work with minimal supervision.
• Full-time 40 hours per week (Mon-Fri).
• Valid driver's license, car insurance, and access to an automobile.

Responsabilités

• Provide clinical support and guidance for members with medical complexity and coordinate care plans.
• Address barriers to health care and advocate for optimal member outcomes.
• Review, assess, and complete medical complexity attestations and clinical oversights.
• Ensure services are delivered in least restrictive settings to meet well-being goals.
• Develop and modify Individual Care Plans with care team involvement.
• Support members and caregivers using an interdisciplinary approach to access services.
• Serve as primary contact for the Interdisciplinary Care Team (ICT).
• Coordinate with ICT and external resources to meet member needs.
