Job summary

Humana seeks a Field Care Manager Nurse 2 in Michigan to assess member needs, coordinate care, and guide families to appropriate resources. The role involves home visits, telephonic assessments, and collaboration with a care team.

Travel is substantial (75–90%), with a 40-hour work week, requiring MI licensure (RN/APRN) and prior case-management experience to support adults and navigate community services.

Qualifications

• Must reside in Michigan (Wayne or Macomb Counties).
• Active Michigan license as RN/APRN, including NP or CNS, with no disciplinary action.
• 2+ years’ experience in Health Care and/or Case Management.
• Ability to travel to homes and community settings for face-to-face assessments.
• Experience with adult population and disease management.
• Knowledge of community health and social service agencies and resources.
• Ability to use electronic information applications & software including EMR.
• Excellent keyboard and web navigation skills.
• Intermediate to advanced Microsoft Word, Outlook, and Excel.

Responsibilities

• Manage a caseload and complete assessments in home, community, or telephonic settings.
• Provide clinical guidance, develop and coordinate care plans for medically complex members.
• Review medical complexity attestations and clinical oversight activities.
• Ensure services are provided in the least restrictive setting while meeting needs.
• Develop/modify the Individual Care Plan with the care team involvement.
• Support members/caregivers using interdisciplinary approaches to access services.
• Serve as primary contact for the Interdisciplinary Care Team and coordinate resources.
