Job summary

HireTalent is seeking a dedicated Field Care Manager RN II (RN Case Manager) to support Medicare/Medicaid members through comprehensive care coordination, clinical assessment, and ongoing case management. This field-based role conducts in-home assessments, develops personalized plans, and coordinates services to achieve optimal health outcomes.

The role requires and rewards independent work, strong communication, and travel across Wayne and Macomb Counties, with a home-office setup and full-time

Qualifications

• Active Michigan RN license with no disciplinary actions.
• Must reside in Michigan.
• Ability to travel throughout Wayne and Macomb Counties for in-home visits.
• Experience with adult populations and chronic disease management.
• Knowledge of community health resources and social services.
• Strong assessment and care coordination skills.

Responsibilities

• Conduct in-home and community assessments for Medicare/Medicaid members.
• Develop and update individualized care plans.
• Coordinate services with physicians, specialists, and caregivers.
• Monitor member progress and adjust plans as needed.
• Identify barriers to access and treatment adherence.
