Job summary

Qualifications

• Active Michigan RN/APRN license with no disciplinary actions.
• Experience with HCBS and LTSS programs.
• 2+ years in healthcare or case management.
• Strong documentation and interdisciplinary collaboration.

Responsibilities

• Assess member needs in home, community, and telephonic settings.
• Develop and coordinate care plans for HCBS/LTSS services.
• Advise on services, address barriers, and advocate for outcomes.
• Conduct InterRAI IHC assessments and NFLOCDs.
• Coordinate with the Interdisciplinary Care Team for member needs.
