Humana is seeking a Field Care Manager Nurse 2 in Michigan to assess member needs, guide families to resources, and coordinate care plans in home or community settings, with telephonic support. You will manage a caseload, provide clinical guidance for complex cases, and work with an interdisciplinary team to optimize outcomes.

The role requires an active Michigan RN or APRN license, 2+ years in health care or case management, and 75–90% travel within Wayne/Macomb Counties.

Qualifikationen

• Must reside in Michigan (Wayne or Macomb Counties).
• Active Michigan RN or APRN license (including NP or CNS) with no disciplinary action.
• 2+ years’ experience in health care and/or case management.
• Ability to travel to homes and community settings for assessments.
• Experience with adult population and disease management.
• Knowledge of community health and social service agencies.
• Proficiency with electronic medical records and MS Office tools.

Aufgaben

• Manage a caseload and complete assessments in home, community, or telephonic settings.
• Provide clinical guidance for members with medical complexity and coordinate care plans.
• Review medical complexity attestations and clinical oversight activities.
• Ensure services in least restrictive settings to maintain well‑being.
• Develop and modify Individual Care Plans with care team involvement.
• Support members/caregivers using an interdisciplinary approach to access services.
• Serve as primary contact for Interdisciplinary Care Team and coordinate with resources.
