Qualifikationen

• Active Michigan RN/APRN license (NP/CNS) with no disciplinary actions.
• 2+ years healthcare/case management experience.
• Experience with HCBS and waiver programs.
• Experience LTSS assessments and Medicaid regulations.
• Ability to document accurately per state/federal requirements.
• Experience collaborating with interdisciplinary teams to assess and plan care.
• Proficient with EMRs and MS Office tools.

Aufgaben

• Manage caseload and complete in-home, community-based, and telephonic assessments.
• Provide clinical guidance and coordinate care plans for complex needs.
• Address barriers to healthcare and advocate for member outcomes.
• Complete InterRAI IHC assessments and NFLOCDs to support eligibility decisions.
• Develop and update Individual Care Plans with care teams and informal caregivers.
• Coordinate with ICT and external resources to meet member needs.
