Job summary

Humana seeks a Field Care Manager, Social Work in Michigan to assess members’ needs, guide access to resources, and support care coordination for LTSS and HCBS. The role emphasizes person-centered, trauma-informed, and culturally responsive care, with significant travel within Wayne and Macomb Counties.

The incumbent will document findings, collaborate with interdisciplinary teams, and monitor progress toward outcomes, ensuring adherence to state and federal requirements.

Qualifications

• Must reside in Michigan and be able to travel throughout the assigned service area, including Wayne and Macomb Counties.
• Active Michigan Social Work license in good standing required (LBSW, LLMSW, or LMSW).
• 2+ years post-degree experience in social work, behavioral health, health care, care management, or community-based services.
• 1+ year experience supporting Home and Community-Based Services (HCBS) or waiver programs.
• Experience conducting LTSS assessments and using planning to support eligibility determinations and care coordination.
• Knowledge of LTSS, HCBS, Medicaid requirements, and person-centered planning principles.
• Experience collaborating with interdisciplinary teams and community partners.
• Strong written and verbal communication; ability to document accurately and timely.

Responsibilities

• Independently determine appropriate actions within guidelines and procedures.
• Monitor patient progress toward outcomes and may create care plans.
• Apply trauma-informed and culturally responsive approaches in the field.
• Coordinate with providers, families, and community organizations.
