Job summary

CVS Health Corporation in Ohio is seeking a Field-Based Care Coordinator to deliver high-quality care management for Medicaid members in the West Region. This full-time role requires travel within the assigned Western Ohio counties to member homes and locations, with a typical 40-hour work week and some after-hours flexibility.

You will coordinate care plans, collaborate with health professionals, and empower families to improve health outcomes across multiple child and family services.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• Proficient with MS Office Suite (Outlook, Word, Excel, SharePoint).
• Bachelor's or master's degree in behavioral health, human services, health services, or public health preferred.

Responsibilities

• Coordinate and implement care plan activities and monitor care plan progress.
• Collaborate with Medical Directors and case managers to resolve issues and improve outcomes.
• Provide coaching and education to empower members to participate in healthcare decisions.
• Act as a single point of contact for members and help close gaps in care.
