Job summary

CVS Health Corporation in Ohio seeks a Care Management Coordinator for a full-time field-based telework role. You will travel within the assigned region up to 50% to member homes and locations, coordinating care plans, educating members, and supporting families to improve health outcomes.

Requirements include at least 2 years in behavioral health or related public-sector human services, a Bachelor's degree (or non-licensed master's), proficiency with MS Office, and willingness to travel.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years using MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
• 2+ years in family systems, case management, child counseling/therapy, child protection, or child development.
• 2+ years with Ohio delivery systems, including local networks and resources.
• Willing and able to travel within the assigned region up to 50% of the time.
• Reliable transportation.
• Willing to work beyond 8am-5pm as needed.
• Bachelor's degree or non-licensed master level clinician.
• Must reside in Ohio.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate members' needs and eligibility and refer to appropriate services.
• Collaborate with multidisciplinary teams to ensure optimal outcomes.
• Provide coaching and support to empower members in health decisions.
• Serve as single point of contact for members and address gaps in care.
• Ensure compliance with regulatory and accreditation guidelines.
