Job summary

CVS Health Corporation is seeking a full-time Field-based Case Management Coordinator in Ohio. The role involves traveling within Central and Southeast Ohio to member homes and locations up to 50% of the time, coordinating care and supporting families in Medicaid-related services.

Strong candidates reside in Central/SE Ohio with a background in behavioral health, case management, and knowledge of local delivery systems.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
• 2+ years in family systems, community systems and resources, case management, child counseling/therapy, child protection, or child development.
• 2+ years experience with Ohio delivery systems, including local community networks and resources.
• Willing and able to travel within the assigned region up to 50% of the time; Some travel to the New Albany office may be required for trainings/meetings.
• Reliable transportation required.
• Willing and able to work beyond core business hours, Monday-Friday, 8am-5pm, as needed.

Responsibilities

• Be clinically and culturally competent with training to manage complex cases in the community across child-serving systems.
• Evaluate members’ needs and eligibility, review benefit plans, and coordinate care planning.
• Coordinate and implement assigned care plan activities and monitor progress.
• Collaborate with Medical Directors and other programs to overcome barriers and achieve optimal outcomes.
• Serve as a single point of contact for members and help address gaps in care and access.
• Ensure documentation and compliance with regulatory and accreditation guidelines.
