Job summary

Qualifications

• 2+ years of experience in behavioral health, social or human services.
• 2+ years of experience with computers and MS Office applications.
• 2+ years in children's mental health, child welfare or related field.
• 2+ years in family systems, case management or related areas.
• 2+ years with Ohio delivery systems and local resources.
• Must reside in the West Region of Ohio.
• Willing to travel up to 50% within the region.
• Reliable transportation and readiness to work extended hours.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Collaborate with case managers, Medical Directors and others to resolve barriers.
• Provide coaching and support to empower member healthcare choices.
• Serve as single point of contact for members and address care gaps.
• Document care management activities per regulatory and company policies.
