CVS Health Corporation is seeking a dedicated Care Management Coordinator for a full-time field-based telework role in Ohio. The position requires traveling within Ohio up to 50% to member homes and locations; candidates must reside in central or southeastern Ohio.

The role focuses on coordinating behavioral health services for children and families, collaborating with MCOs, vendors, and ODM to ensure comprehensive care plans, including regulatory compliance and quality of care improvements.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or related public sector field.
• 2+ years experience in areas like family systems, case management, or child development.
• 2+ year experience with Ohio delivery systems and local networks.
• Willing and able to travel within Ohio up to 50% of time; some travel to New Albany office for trainings may be required.
• Bachelor's degree or non‑licensed master level clinician; must reside in Ohio.

Responsibilities

• Provide care coordination and education to members.
• Collaborate with care managers, Medical Directors and other health programs to optimize outcomes.
• Coordinate and monitor care plan activities and progress.
• Identify and address quality of care issues and barriers to meeting goals.
