Job summary

Molina Healthcare is seeking a qualified care manager to support LTSS coordination and care management for high-need members. You will collaborate with a multidisciplinary team to build and monitor care plans, ensure timely assessments, and facilitate waiver enrollment and services.

Role involves travel (25–40%), strong communication, and knowledge of community resources. LCSW/LMSW/APSW licenses and relevant LTSS experience are preferred.

Qualifications

• 2+ years in health care, with 1+ year in care management/LTSS
• Active professional license in state of practice
• Valid driver's license and auto insurance
• Able to work independently with minimal supervision
• Experience with diverse populations and behavioral health settings
• Knowledge of community resources

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including home visits if required
• Coordinate waiver enrollment/disenrollment processes
• Develop and implement care plans with members, caregivers and health professionals
• Monitor care plan effectiveness and adjust interventions as needed
• Promote integration of behavioral health and LTSS services
• Assess medical necessity and authorize waiver services as appropriate
• Identify barriers to care and address psycho/social, financial, and medical obstacles
• Travel up to 25–40% locally as required by state/contractual rules
