Job summary

Molina Healthcare is hiring a Care Coordinator to support LTSS and care management activities. The role collaborates with a multidisciplinary team to coordinate integrated member care across the continuum, ensuring progress toward outcomes and contributing to cost-effective care.

Responsibilities include member assessments, waiver enrollment, care plan development, monitoring, and coordinating with caregivers and health professionals. Travel up to 40% may be required depending on state contracts.

Qualifications

• At least 2 years of health care experience, including 1 year with persons with disabilities/chronic conditions LTSS, and 1 year in care management or equivalent.
• Licensure/certification required only if state contract/regs require; if licensed, license must be active and unrestricted in state of practice.
• In some states, a bachelor's degree in a health care related field may be required.
• Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job travel requirements.
• Demonstrated knowledge of community resources.
• Ability to work in diverse settings with varied populations and personal situations.
• Ability to work independently with minimal supervision and self-motivation.
• Strong verbal and written communication skills.
• Proficiency in Microsoft Office and ability to navigate online portals/databases.
• In some states, at least one year of experience working directly with individuals with substance use disorders.

Responsibilities

• Completes comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans, including a waiver service plan with members, caregivers, physicians and others.
• Monitors care plan effectiveness and documents interventions and goal achievement.
• Promotes integration of services to enhance continuity of care.
• Assesses medical necessity and authorizes waiver services.
• Evaluates benefits and funding sources.
• Facilitates ICT meetings for approval/denial of services.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Addresses psycho/social, financial, and medical obstacles to care.
• Identifies incidents and develops prevention plans for member health.
• Collaborates with licensed care managers/leadership as needed.
• Travel 25-40% may be required.
