Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite applications.
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or similar.
• 2+ years experience in case management, child and family counseling/therapy, or related areas.
• 2+ years with Ohio delivery systems and local networks.
• Willing and able to travel within Northeast Ohio up to 50% of time.

Responsibilities

• Coordinate and implement assigned care plan activities and monitor progress.
• Collaborate with case managers, supervisors, and medical directors to address barriers to goals.
• Act as single point of contact to assist members with accessing care and resources.
• Ensure documentation aligns with regulatory and accreditation guidelines.
