Qualifications

• Willing and able to travel within the assigned region up to 50% of the time; Ohio delivery systems knowledge preferred.

Responsibilities

• Perform comprehensive evaluation of referred members, assess needs and benefits, and propose case resolution strategies.
• Coordinate and monitor care plans, ensuring progress and alignment with member goals.
• Collaborate with case managers, supervisors, and health teams to optimize care quality and engagement.
• Maintain documentation in line with regulatory and accreditation requirements.
