Job summary

CVS Health Corporation is seeking a Case Management Coordinator in Ohio. You will assess complex cases, coordinate care plans, and support families through community-based services. This role requires travel within Northeast Ohio up to 50% and a focus on coordinated care across child-serving systems.

The position emphasizes collaboration with teams, utilization review, and empowering families to improve health outcomes. A behavioral health background and Ohio familiarity are essential.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services.
• 2+ years experience in family systems, community systems and resources, case management, child and family counseling/therapy, child protection, or child development.
• 2+ years of experience with Ohio delivery systems, including local networks and resources.
• Willing to travel within the assigned region up to 50% of the time; some travel to New Albany office may be required.
• Reliable transportation; mileage reimbursed per policy.
• Willing to work beyond core hours, Mon-Fri 8am-5pm as needed.

Responsibilities

• Be clinically and culturally competent in managing complex cases in the community across child-serving systems.
• Evaluate member needs and eligibility; propose case resolution and care planning.
• Coordinate and monitor care plan activities and progress.
• Consult with case managers, Medical Directors, and other programs to overcome barriers to goals.
• Work with Child and Family Teams; escalate quality of care issues as needed.
• Provide coaching and support to empower members in health decisions.
