Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health or related field providing community-based services.
• Willing and able to travel within the region up to 50%.

Responsibilities

• Coordinate care management and education for members to support healthcare outcomes.
• Evaluate referred members' needs, eligibility, and develop care plans.
• Collaborate with care teams to ensure quality of care and appropriate services.
• Monitor, document, and communicate progress in care management processes.
