Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time field-based telework role in Ohio. Travel within Northeast Ohio up to 50% of the time to member homes and locations is required.

Strongly preferred reside in counties including Cuyahoga, Lorain, Medina, Summit, Lake, or Geauga. The role emphasizes care coordination, education for members, and collaboration with care teams to improve health outcomes.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint)
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or public sector human services
• 2+ years experience in case management, child and family counseling/therapy, child protection, or child development
• 2+ years experience with Ohio delivery systems, local networks and resources
• Willing and able to travel within the assigned region up to 50% of the time

Responsibilities

• Manage complex cases in the community across child-serving systems
• Evaluate members' needs/eligibility and coordinate care plans
• Collaborate with case managers, supervisors, and health professionals
• Monitor and document care in compliance with regulations
